You will get a contract form with your confirmation email when you enroll

online - please bring to class.  OR you may follow the steps below or complete

the contract form at the school prior to class.

2. PRINT CONTRACT

This will be attached to your Confirmation Email from PDS

3. FILL OUT COMPLETELY

4. Mail, Email (Scan), or

Bring it on the Day of Class

(students under 18 MUST have parent's signature)
